I being parent/guardian of the said child understand that whilst every precaution will be taken to ensure the good welfare and protection of my child, One Another Church, its staff and volunteers acting on behalf are hereby released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to the child and their property. In the case of an emergency, I hereby give permission to the First Aid Staff to ensure proper treatment for my child. I understand that every effort will be made to contact me before instituting such procedures. I agree to pay all such doctor, ambulance and hospital fees incurred on behalf of my child. I have attached a list of any health information about my child that the First Aid Staff need to know.
I recognise that being a part of a community involves mutual care and consideration therefore agree that unacceptable behaviour may result in my child/ren being sent home at my expense and/or being temporarily or permanently excluded or prohibited from attending One Another Church ministries.
I understand that for the safety and privacy of all children, children are not permitted to bring mobile phones or recording devices into One Another Church ministries. We encourage children not to bring any devices or personal belongings to our programs, however, there will be a space made available for children to place any devices/personal belongings upon entry. One Another Church ministries takes no responsibility for any items that are lost or stolen.
I give my consent that my son/daughter may participate in any activities they choose.
I understand that in the event of fire drill or evacuation procedures, my children may need to be taken offsite.
I understand that every effort will be made to provide a safe environment for my child/ren to participate in; however, in signing this form I authorize the leaders, in the event of an emergency, to obtain at my expense, any medical, ambulance or similar services considered necessary by the leaders.
I recognize that my child/ren will receive biblical teaching consistent with the doctrine of Australian Christian Churches, training and prayer while attending children’s ministry related activities.
One Another Church will provide supervision at a parent pick-up point at the venue. No child will be allowed to leave the venue until they are picked up by a parent/guardian. Where parents will not, for any reason, collect their children from the supervised pick-up point, then One Another Church, its employees, servants and volunteers, as organisers of One Another Church ministries, will contact the police. In this case, we take no responsibility for any child leaving the supervised site for whatever reason.
